The setup, spoiler-light
Modern Warfare 4 picks up after Price killed General Shepherd at the end of Modern Warfare III. Task Force 141 has been disbanded. Price is an operator-turned-outlaw, and Ghost and Gaz are sent to capture or kill their former captain. Laswell keeps a line open, but even she is unsure how far Price has gone.
Meanwhile, North Korea launches a full-scale invasion of the peninsula. You fight it from two angles: a young South Korean squad trying to hold Hajin City, and Price's off-book hunt for a weapon being delivered to a broker known only as The Archer. The campaign cuts between the two threads and brings them together in the final act.
That structure is why the campaign feels bigger than recent entries. It is not one squad walking down a corridor; it is a war story and a manhunt running in parallel, with a shared cast that keeps crossing over.
The mission flow
The first half alternates between the invasion and Price's errands. Hajin City falls, Paris becomes a cartel warzone, Mumbai hosts the handoff, and a North Korean palace coup changes who owns the war. The second half is the counterattack: a covert raid, a behind-enemy-lines survival mission, an airbase assault, the retaking of Hajin, and a finale on the Russian border.
Two missions break the formula. Stranded is an open-area survival mission with optional exploration, 26 supply crates and a large number of collectible notes. Entrenched is a large-scale assault on a nuclear power station and was the first campaign mission ever playable in a Call of Duty beta. Both are worth playing slowly.
The full mission list, objectives and rewards are on the 16-mission walkthrough. If you want the ending and the death list without playing, the ending explained page has full spoilers.
Six difficulty settings
| Difficulty | Unlock | What changes |
|---|---|---|
| Recruit | From the start | Lowest enemy damage, generous health |
| Regular | From the start | The intended first playthrough |
| Hardened | From the start | More aggressive AI, tighter ammo |
| Veteran | From the start | High damage, limited checkpoints |
| Realism | After one completion | No HUD, lethal damage, realistic ballistics |
| YOLO | After Realism completion | One life for the entire campaign |
YOLO is the headline addition. Any death, failed objective, civilian kill or friendly-fire incident sends you back to Mission 1. That means a minimum of two full playthroughs before you can even attempt it, and a third if you want the achievement. Do not start with YOLO.
The campaign also uses a Shockwave system: explosions knock you down instead of killing you outright. On lower difficulties it keeps the story moving through set-piece moments. On Veteran and Realism, do not treat it as a free pass — the follow-up damage usually is not.
Completion rewards
Every mission unlocks something for Multiplayer or DMZ: eight operators, calling cards, emblems, loading screens and Double XP tokens. The operator unlocks are the ones that matter for players who want a specific look without spending COD Points. The full reward-by-mission table is in the walkthrough.
Campaign rewards carry into the full game even if you finish during early access. Multiplayer rank and weapon levels do not; those start at zero on October 23.
Collectibles and completion
There are 64 notes spread across Following Orders, Stranded and Escape, plus 26 yellow supply crates in Stranded. Several crates sit behind locked containers and bunkers that require codes found in the mission. The campaign does not gate story progress behind collectibles, so you can finish the story first and clean up with mission select.
For a completionist run, budget 14–16 hours and at least two playthroughs: one on any difficulty to unlock Realism, then one on Realism to unlock YOLO.
What to know before mission one
- Play the campaign before the multiplayer grind. Spoilers will be everywhere on launch day, and the campaign's biggest moments are character deaths.
- Do not skip the South Korean missions. They carry the emotional weight and set up DMZ's Hajin zone.
- Use headphones. The campaign leans on directional audio for ambushes and civilian crowds.
- Check your install. Campaign is a separate pack on PS5 and Xbox; if you only preloaded multiplayer, you cannot start early access.
- Expect a patch. Campaign early access builds have received updates; let the shader and patch process finish before launching.
